Patient Information
At Salem Memorial District Hospital (SMDH), we are committed to every patient, every encounter, every time. We do not deny services based on a person’s race, color, sex, national origin, disability, religion, sexual orientation, or inability to pay. We also offer discounted fees for qualifying patients under our Financial Assistance Program, available through the Financial Assistance option in the left-hand menu.
Patient Portal Access
If you have recently been seen at SMDH, you may have signed up for a Patient Portal account. - If you received an invitation, please follow the instructions in your email to set that up.If you do not yet have a Patient Portal account, be sure to ask about it when requesting your medical record. You may also print the Medical Records Request Form if you have not yet signed up to share your records through the Health Information Exchange (HIE).
Health Information Exchange (HIE)
SMDH supports the secure sharing of medical records through the Missouri Health Information Exchange, giving physicians immediate access to your information when: - You are in an emergency situation, or - Your doctor needs your records promptly for better care.
