ToRCH and Help for Individuals

Salem Memorial Hospital, primary care and behavioral health providers and community-based organizations offering social services are working together to improve community health for Dent County’s Medicaid population.

If you need help with housing, food, transportation, etc., look for available community resources or complete an assistance request form through Unite Us. Local community-based organizations that are part of the Unite Us network will see your request. Eligible ToRCH participants can expect to hear from an organization that can help within a week.
