Swing-Bed (Transitional Care) Program

What Is Transitional Care?

Transitional care, also called swing-bed care, is designed for patients who need additional time to recover after a hospital stay.

This program helps patients transition from hospital-level care to rehabilitation and skilled nursing care — without needing to leave the hospital. It allows patients to continue their recovery close to home in a supportive, familiar setting.

Transitional care can also help patients who were hospitalized away from home transfer closer to home to complete their recovery.

At Salem Memorial Hospital, our transitional care program supports your recovery journey — whether you are preparing to return home independently or with the support of outpatient therapy services.

Who Qualifies?

Medicare and most insurance plans cover transitional care under the Skilled Nursing Facility benefit.

To qualify:
You must be admitted to a hospital as an inpatient (not under “observation status”)

You must have a hospital stay of at least three consecutive nights within a 30-day period


Once these requirements are met, you may be admitted to our swing-bed program from any hospital.

Who May Benefit?

Our Swing-Bed Program may benefit patients who:

1. Need rehabilitation or therapy following orthopedic surgery, such as joint replacement

2. Are recovering from a stroke but do not qualify for admission to an inpatient rehabilitation center

3. Require IV therapy for wound care

4. Need to regain strength and mobility after illness, injury, or surgery


Will Insurance Pay for My Stay?

Medicare covers:
The first 20 days of care at 100%
Up to 80 additional days at 80% coverage if more time is needed


Most secondary insurance plans will pay the remaining 20% after the first 20 days.

Many private and government-funded insurance plans require prior authorization before covering a swing-bed stay.

Our Transitional Care Coordinator and Billing Department will verify your benefits and explain your coverage in detail.

Admission and Discharge

When skilled nursing care is needed, your physician or hospital will send a referral. Our care team will work with you to coordinate your admission.

We encourage family and friends to visit frequently. They play an important role in recovery and are welcome to participate in care meetings to stay informed and involved.

As you meet your rehabilitation goals, our team will assist you and your loved ones in planning for a safe and smooth return home.
